Aetiology and the mode of presentation of congenital hypothyroidism – a 10 year single centre experience in a tertiary care centre in Sri Lanka

Siriwardhane Dinendra , Atapattu Navoda

Introduction: Congenital hypothyroidism (CH) is defined as thyroid hormone deficiency which is present at birth. Thyroid hormones are crucial for early neurodevelopment. Untreated severe CH results in major neurological deficits. Therefore, if treatment is initiated early, these deficiencies can be prevented. Newborn screening for CH was first initiated in Sri Lanka in September 2010.Methods: A retrospective cohort study...

Vitamin D supplementation for chronically ill patients: where are we?

Atapattu Navoda , Shaw Nicholas , Hogler Wolfgang

Introduction: ‘At risk’ groups for vitamin D deficiency have long been identified, including the chronically ill. According to guidelines published by the European Society of Endocrinology (ESPE, 2002), the American Academy of Paediatrics (AAP, 2008) and the Endocrine Society (ENDO, 2011), vitamin D levels should be measured in chronically ill or at risk patients. We aimed to test the current knowledge of these guidelines amongst consultants in a large tertiary hospi...

What defines vitamin D deficiency biochemically in children?

Atapattu Navoda , Shaw Nicholas , Davies Paul , Hogler Wolfgang

Background: The level of 25OH vitamin D (25OHD) which separates deficiency from sufficiency is heavily deabted. With decreasing 25OHD levels, typical biochemical derangements set in, such as increasing parathyroid hormone (PTH) and alkaline phosphatase (ALP) levels, and decreasing phosphate and calcium levels. In adults, serum concentrations of PTH reportedly start to rise when serum 25OHD levels drop below 30 μg/l. Such data are scarce in children....

Effect of patient choice and hospital tracking on short term growth in children treated with GH therapy

Atapattu Navoda , Casey Angela , Prior Julia , Howarth Isobel , Zia Rani , Akhtar Saeeda , Davies Paul , Kirk Jeremy

Introduction: Most (89%) UK units offer some form of free patient choice for new paediatric patients commencing GH therapy. Initial data indicates that patient choice improves adherence, resulting in improved growth (height velocity) short-term.Objective: To compare outcome measures between patients offered free choice and/or hospital supply (including home services and adherence tracking assessed using ampoule counting) with GH therapy with those who di...

Plasma glucose and gut hormone responses in obese children after variable resistant starch and protein content

Suntharesan Jananie , Atapattu Navoda , De Silva Harendra , Jasinge Eresha , Ekanayake Sagarika , Dunseath Gareth , Luzio Steve , Premawardhana Lakdas

Introduction: Resistant starch (RS) has beneficial effects on postprandial glucose metabolism in both animals and adults. Hitherto, it has not been studied in children. The long-term effects of RS in reducing obesity and improving metabolic profiles need to be investigated in children. Objectives: Our objective was to compare serial plasma glucose, insulin, gut hormone profiles and satiety scores in obese children after meals, containing increasing amoun...

Introduction: There is no unified approach in clinical practice regarding the medical management of congenital adrenal hyperplasia (CAH), despite existent international guidance. We aimed to explore geographical and temporal variations in the treatment with glucocorticoids and mineralocorticoids of patients with CAH.Methods: We collected data recorded by 33 centres from 16 countries in the I-CAH Registry. We analysed patient visits between 1982 and 2018,...

Background: The reported occurrence and management of acute adrenal insufficiency–related adverse events in children vary widely between centres and may depend on available resources. Methods: Real world data from the I-CAH Registry from 44 centres [32 from high income (HIC) and 12 from low/middle income (LMIC) countries] and a total number of 607 children were linked to the results of a health care survey of local r...
